Health officials are investigating whether meeting documents contained “inappropriate” information about a woman who called 111 but died before receiving a call back from a doctor.
The incident, on December 22 last year, was mentioned in a report to January’s board meeting of the Croydon Clinical Commissioning Group (CCG).
Details outlined in the document were sparse and her name, age and address were not made public but the CCG is still concerned it may have put too much information in the public domain.
In papers produced for yesterday’s meeting of the CCG it said: “The CCG has declared an SI (serious incident) following an inappropriate level of detail being presented in a governing body report.
“The root cause analysis investigation is now in progress.”
Harmoni, which runs the NHS 111 service in south west London, is leading the investigation into the 33-year-old woman’s death.
Its investigation is still ongoing.
